Do I need a physical therapy referral or prescription from a physician?
In Georgia, you can be seen by a physical therapist without a physician referral for 8 visits or 21 days, whichever comes first. Only once one of these qualifiers is met will we require that you obtain a physician referral/prescription for physical therapy. If you have been seen by a primary care physician, orthopedist, surgeon, etc. within the last year, this is generally easy to obtain by simply calling their office.
Referrals/prescriptions can be faxed to (404) 882-3151 or you can bring in a paper copy.
What forms of payment are accepted?
Accepted forms of payment include Visa, Visa Debit, MasterCard, Debit MasterCard, American Express, Discover, Diners Club, UnionPay, Apple Pay, and Google Pay. Payments from HSA, HRA, and FSA accounts are also accepted for medically necessary services.
When paying out-of-pocket, payment is expected at time of service. A Good Faith Estimate is available upon request. You can also give us a call for further information about pricing.
What is your cancellation policy?
Your appointment time is reserved just for you. As such, we ask that all patients give 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice or who miss their appointment will be assessed a $50 late cancellation/no show fee. We reserve the right to waive this policy at our sole discretion.
Where do I park?
We offer validated parking in the West Court Square garage.
Enter 140 Swanton Way into your GPS. It will look like you are driving into the MARTA bus station, but this is correct. Pull into the garage through either lane by taking a ticket. As you pull in, note the building entrance is to your right, though you can park anywhere in the garage. Follow the signs to Clarity Fitness until you reach the big blue front door.
*During the day, you can walk through the two sets of glass doors. After hours, you will have to walk around the perimeter of the building.
What should I wear?
First and foremost, you should wear something you are comfortable in that does not restrict your movement. Athletic clothing such as a t-shirt or tank top with shorts, leggings, or joggers is recommended.
If you are coming in for a running related injury, please wear either shorts or leggings as well as your usual running shoes so we can accurately assess your lower extremity movement.
If you are wearing loose fitting shorts, please be mindful that you are wearing appropriate undergarments!
